Dr. Janaki Gopalan, Specialist Gynaecologist, Welcare Hospital responds:

Dear Doctor,
My husband's motility rate is 22%to 25%. Is this normal or not? Please tell as we are planning for a baby. Shama, Kuwait

If the count is all right, this motility should suffice. However if you are having problems in conceiving, please ask your husband to see an Urologist for a check up.

Dear Doctor,

I have piles problems (it's hereditary). Now that I am pregnant I find it very difficult to sit after passing my stool as it pains a lot. I was taking some ayurvedic tabs earlier (stopped now coz of pregnancy) Can I still continue as that allows me to pass stool easily. Mariyam, India

I do not know much about what the ayurvedic tablets contain. You are allowed to take some stool softeners - Movicol , duphalac syrup or isapghul

Dear Doctor,
I have heard that a woman's urinating frequency increases when she is pregnant. I am pregnant and I go to urinate 5-6 times in 24 hours. Is this normal? Please advice. Rahat, Kuwait

If your urine does not show an infection and if your blood sugars are all right, increase in frequency of micturition is normal in pregnancy.

Dear Doctor,
I am 31 years old having one daughter of 6 years old. I am having irregular periods sometimes once in 3 months. In this case how we will calculate the ovulation time and how can I correct the irregularity? Is the treatment very costly? Sreelekha, Dubai

You need to see a gynaecologist - as there may be an underlying hormonal disorder which will need to be treated and will let you conceive. If you are over weight please loose weight and achieve normal levels.

Dear Doctor,
During my periods I have lumps of blood like clots. Is this normal? Amy, Essex

Please see your GP and ensure that there are no causes for the clots and heavy periods. If your doctor says that the uterus and ovaries are normal, and if you are not anaemic, then you may only need to take tablets at periods to make it lighter and without clots.

Dr. Mohd. Ali Bouhadiba, Specialist Obstetrician & Gynaecologist, Welcare Hospital responds:

Dear Doctor,
I am taking clomid 0.5mg, for the past 4 months. Since two months I am also taking metaphage and dopergin. For three cycles of clomid I ovulated but 4th cycle I did not ovulate. Also I did not conceive for last 3 cycles of clomid. I have pcod. Kindly advise whether I can get pregnant. My Fallopian tubes are ok. What I should do to get pregnant? Mano, Dubai

Once you take an efficient treatment, you return to normal. Out of 100 normal women only 50% will get pregnant in 6 months and it will take up to one year to reach 90%. You should be more patient and carry on with your treatment for few more months, since you are ovulating with it. Has your husband done a spermogram?

Dear Doctor,
My cousin got married in Jan 2004. After her marriage she got her 1st periods but after that till to date she hasn't got her periods. She has done pregnancy tests and Ultrasound but they all show a negative report of pregnancy. Is this normal? Am not sure about her age but she must be between 18-20. Naila, Dsm, Tanzania

Was it the 1st period of her life or after marriage? If she is between 18 and 20 and not getting her periods regularly, she should see a doctor.

Dear Doctor,
I have not been able to conceive for last five years. When I went for check up they said that I have PID, which resulted in tubal blockage (both), can you suggest me some remedies for the both. Tshering, Bhutan

Unfortunately PID results often in tubal blockage. You must first get rid of the PID specially if this has been going on for a while. Once the pelvic tissues become healthy you may have the choice between tubal surgery if the tubes are not too damaged or IVF.

Dear Doctor,
What's better contraceptive tablet 'novelon' for wife or vasectomy for husband Tanusree Bhattacharyya, Jamshedpur

Overall it appears that vasectomy is safer, but it is really a matter of personal choice that should be discussed between the two of you. Do not forget that the pill is a reversible contraception while vasectomy is not.

Dear Doctor,
My mother is a 45-year-old woman who examines her breasts once a month after her period for any lumps and irregularities. Recently during her routine exam she noticed 2 lumps on her right breast, she went for a mammography and the doctor told her that those lumps are non-cancerous cysts. Do these have to be removed? Are there any chances of these cysts turning into cancerous ones? Lara Castellano, Sharjah

Your mother is a very cautious person and I wish all women did the same. If the cysts are palpable it means that they are of a reasonable size. I would advise puncture with a fine needle with control of the floating cells. The vast majority of cysts are benign unless if the cyst wall looks thick.

Dr. Bindiya Magoon, General Practitioner, Welcare Hospital responds:

Dear Doctor,
I have 2 years 3 months old daughter. She coughs everyday only in the early morning hours. We consulted a doc when we were in India and he mentioned that we get a allergy test done and after that told us not to stop giving her things which she is allergic to, like milk, fish, almonds, wheat. I do give her wheat in the form of bread but very rarely. She was on a course of med for 3 -4 weeks but we found no improvement. Please can you suggest a remedy. This problem has been persisting since Dec 2003. Sneha, Sharjah

The history provided by you is not enough for me to make a diagnosis. However, in children with early morning cough we have to consider the possibility of bronchial asthma. You should consult a specialist paediatrician to rule out Bronchial Asthma.

Dear Doctor,
I am Arshad in Islamabad and want to inquire from a Medical Specialist the bad effects of intestinal worms longer infection on human stomach and lunghs. On knowing it and consulting a doctor, my wife, 28, has done two courses of relevant medicines for the intestinal worms; It seems that the worms' infection started about two years ago and she started suffering from bad digestion and even difficulty in breathing with severe cough. Although, she has completed two courses of Vermox & Flagyl but the worms, though dead, sometimes pass in stool. And even more important is to state here that she still suffers from in-digestion with difficulty in breathing. My question is that kindly let me know what side effects or what harms these intestinal worms might have done to the stomach and lungs and what is the relevant treatment. I would be grateful to the Medical Specialist and the Women One as well. With best regards, Arshad, Islamabad

Vermon is a good medication and is usually adequate to eradicate worm infestation. However if careful hygiene in handling food and water is not maintained then there could be reinfestation with worms.

If your wife still suffers from indigestion and difficulty in breathing she should have a repeat stool examination and an eosinophil reinfestation. Once deworming is done it is very rare to continue having indigestion and other causes for indigestion and breathing difficulty, should be sought.

Dear Doctor,
Can I go for the laser treatment for my eyes, as I have high power. Is it advisable? What treatment shall I go for? Any sideeffects for that? Please suggest. Rafath Masood, Dubai

Laser treatment is now very popular, for correction of refractive error. However every patient cannot have laser surgery. Multiple factors need to be taken into consideration before deciding whether you will benefit from laser correction or not. These include age, nature of error, stability of error and other things.

It is best to see a refractive surgeon to decide if it will suit your kind of refractive error. Also there are several side effects of the procedure, including over correction and under correction and these again need to be addressed by the concerned surgeon.
